肺泡微结石症一种罕见病。两肺呈弥漫性、均匀分布的肺泡砂粒状病变。病因不明,往往有家族性。本病发现时多无症状,且病情进展极为缓慢,经过十几年甚至几十年,最终多发生呼吸衰竭和肺源性心脏病。病因与病理 本病有家族性。可起病于胎儿期,多数报告认为起病于幼年期,临床发病年龄介于30~50岁之间,两性发病率无明显差别。病因未明,与下列因素有关:①有家族性。中国报告的22例中有家族性的占17例,卡弗里分析的68例中,34例有家族史,且患病者几乎均为同胞,故推测本病有家族性。②由于本病的家族性,暗示病人有先天性代谢障碍,但血清钙与磷的水平均在正常范围,且身体其他部位无钙代谢异常,故推测是局部代谢障碍,钙在碱性环境中容易沉淀,可能由于某些原因,使肺泡界面呈碱性,因而使磷酸钙与碳酸钙易于沉淀。③由于超敏反应损害肺泡表面,导致渗出与钙化。本病早期肺泡壁完全正常,晚期可见间质纤维化,使肺泡壁增厚。肺功能的改变视微结石数量的多少与间质纤维化程度而定。虽然 X射线胸片示广泛肺内病变,两肺功能检查可以正常或几乎正常,到晚期则有肺活量减少,弥散功能减退与通气血流比值失调,气体不能正常交换,身体缺氧,使右心负担增加,终至发生肺源性心脏病,呼吸衰竭。尸检见病肺变得坚实,重量明显增加,放置水中下沉。肺切面可见无数砂粒状小体弥漫分布于两肺,其大小介于0.01~3mm。化学分析,其成分主要为钙与磷,不含硅与铁。临床表现 初期病人多数无自觉症状。晚期病人出现活动后气短,且逐渐加剧,伴咳嗽,咯少量痰。继而出现紫绀。初期无阳性体征,晚期有杵状指及肺心病体征。诊断和治疗 本病多为体检或其他原因被偶然发现,往往无症状, X射线表现极为典型。呈两肺弥漫性细砂状、边缘极为锐利的小点状阴影,分布比较均匀,以中下肺野较密集,可使心缘与膈肌被遮盖而不显影。在局部过度曝光的放大胸片,此种细小点状阴影更为典型,边缘极为锐利。 X射线表现病变非常广泛,但完全没有或仅有轻微的临床症状,此种不相协调的现象是本病特征。若有家族史则诊断并无困难, 有时需与尘肺、 结节病、真菌感染、特发性肺含铁血黄素沉着症及粟粒型肺结核鉴别。本病无特效治疗,皮质激素治疗无效。病人多死于呼吸衰竭及肺心病。
